| Salina Residency Practice Recaps a Successful First Year of PCMH Transformation |
It's been just a year since rural Salina Family HealthCare Center and the Smoky Hill Family Medicine Residency Program launched their medical home transformation initiative with the help of TransforMED.
They made significant health information technology improvements for efficiency, quality and safety. They implemented an innovative "Four Team Concept". Recently on Delta-Exchange, TransforMED's online, collaborative network, DX member Ken Bertka launched an interesting and valuable discussion among the social network's clinicians and thought leaders about the latest "meaningful use" criteria. Ken also posted documents that summarized the proposed criteria for physicians offices.

| Blue Cross and Blue Shield of Kansas City and TransforMED Join with Primary Care Physician Groups in Medical Home Pilot |
Blue Cross and Blue Shield of Kansas City and TransforMED, a wholly owned subsidiary of the American Academy of Family Physicians (AAFP), have introduced a pilot program in which 13 primary care physician groups in the Kansas City area will adopt a new model of health care known as the patient-centered medical home.
David Gentile, president and CEO-elect of Blue Cross and Blue Shield of Kansas City said, "The undertaking of this pilot program is a critical initiative for our company."
